Early Intervention Service “Il Bosco“, Its Integrated Approaches and Service
Development Background in Japan

Background/Objective: In the last 20 years, a number of studies have supported the view that the
earlier detection and care of psychosis can lead to a better outcome. However, most of these studies
were conducted in research settings. Due to the lack of early intervention service specified for
youth generation, “Il Bosco“, day care center, and The Youth Clinic were founded aimed for early
intervention in the Toho University Omori Medical Center in Tokyo.
Method: The unit is run by a strategies of Optimal Treatment Program (OTP) by Ian R.H. Falloon, a
multi-disciplinary team approach plus the cognitive remediation-oriented rehabilitation method.
The service model includes early detection and intervention, repeated assessment and
psychoeducation. Treatment strategies consist of optimal pharmacotherapy by atypical neuroleptics,
cognitive function trainings, cognitive behavioral therapy, and job coaching as a final treatment
program.
Result: People with at-risk mental state exhibit cognitive deficits although they usually demonstrate
superior performance. As cognitive impairments are significantly related to social functioning,
cognitive training focused on divergent thinking in early stages of psychosis may be preferable.
Conclusion: The details of the program and the effectiveness will be presented and discuss about the
group and individual intervention for young people with early psychosis.
